Sprint and Cingular work well, as we have towers for them here. What service do you have?
We had no problems using our Sprint service while there...though we did have some slight service interuptions on STJ when we were at Leinster Bay. Otherwise, all was good! 🙂
My Verizon phone worked well on St John; but surprisingly did not work at the Marriott on St. Thomas. It worked at the St Thomas airport and in town, though.

Beware of roaming charges with Verizon. Also, when using Cingular, make sure you are not on Boat Phone out of the BVI, as the roaming charges will be killer. Places like Drake's Seat on STT are notorious for picking up Boat Phone on Cingular. Sometimes on the East End as well.
I had AT&T service while in St. John last year and it was considered a local call! SunCom has bought them out and told me this will be considered local as well. I used my phone numerous times a day last year since I have teenagers and had not unusual charges on my invoice! Good luck.
My last trip 4/06 my cell phone worked on St. Thomas and St. John. I had at the time Cingular,I'm hoping now that switched to Sprint the same will happen on my next trip. If you don't have Sprint or Cingular you may want to think phone cards.
